Lives Being Saved By An Obamacare Provision - Duration: 8:33.

So, Sheree, embedded in the Affordable Care Act ... I think people know the broad strokes,

and I would say I think America is far more conscious of the broad strokes of what the

Affordable Care Act … we should say the patient protection of the Affordable Care

Act because I think people are becoming more and more aware of the patient protections

involved in what we now call Obamacare.

Within the major pillars, the expansion of Medicaid, the regulations on what constitutes

actual health insurance, the baseline for these things, and then the subsidies involved

and the exchanges, there are a lot of different pilot programs, and that's the one that you

write about that is taking place in Missouri.

Welcome to the show.

Thank you.

Thank you, Sam.

I appreciate that.

Tell us about this story in Missouri.

The story in Missouri is actually a reflection of many things that started happening around

the country that were funded by the Affordable Care Act several years ago, actually.

2009, 2010, 2011.

This program was an integrated care experiment that was funded for Medicaid patients.

Patients that are often called the 5-50.

The five percent of patients who use 50% of medical care, and can cost in some communities

as much as $10,000 a year in ER costs and hospital costs.

In this case, people who also were struggling with various mental illness.

These types of innovations, these types of programs were a key part of the ACA.

This one, I believe, was done through the Social Security Administration.

There are others that were largely done through the Centers for Medicaid and Medicare Innovation

Center, which is still alive, but there is question about how long it may be.

There were efforts to reform payment models and give people better care.

All right.

Let's start with this 5-50 designation.

I've heard of that dynamic a lot more as of late.

For many years, we heard that basically 80% of health care costs are born by 20% of the

population.

This is, I guess, a subset of that 20% population, which bears an even greater percentage of

cost.

Let me ask you this, and then I want to go through what that profile of a person, what

would cause that person to be requiring so much care, but when we talk about the United

States as being much more expensive in terms of health care per capita, is it because that

five percent exists and doesn't exist in other situations?

Is it because the care for that five percent is that much more expensive?

Is it because we lack preventative care that would undercut that?

Does it, I guess, maybe speak to the success of this program as where the delta is between

that five percent in this country and that five percent in, say, Canada?

Yeah.

I would like to say in this case that five percent is usually defined by two or more

chronic health conditions.

Say they have heart disease, diabetes, and hypertension.

In the case of this program, they are also struggling with a serious mental illness.

That is even if it's depression, it's serious enough to require medication.

Washington Monthly Magazine specifically asked me to investigate and to look at programs

for that group.

That's how they're defined, but these people are, of course, all over the world.

The big answer is to why it costs too much, that is the billion dollar question.

I suspect that it is how we care for people in the United States, how we charge for care,

and the lack of a support network for people who really need it, which is what these programs

reveal, which is what the difference is in the health indicators and the health quality

and the life quality of these people when they do get that support.

But it's a very special kind of support.

It's very rare in the United States.

It might be that that support is more available in a country where there might be universal

care, or where social services might be easier to access.

In this particular case, all of these people are Medicaid patients.

It is a fascinating story.

You focused on a woman, Pat Powers, and her treatment at the Crider Health Center in St.

Charles, Missouri.

You really do get a sense of what can go wrong when you have a patient who is taking so many

different medications, but they're coming from so many different sources in some ways.

Yes.

She had a mental health provider who had no relationship with her primary care provider.

They didn't know each other.

She was initially trying to juggle several part time jobs to hold herself together, which

was making her anxiety disorder and depression and other psychological issues more difficult

to manage.

On top of that, she was trying to manage diabetes and weight and medication for her behavioral

disorders all on her own.

She eventually couldn't deal with the job, which meant not being able to keep up with

the insurance.

I think she might have briefly lost housing though she has housing now.

It just came apart.

It requires a great deal of attention.

One of the care providers that I interviewed, Nancy Gongaware, who was a nurse navigator

in this system said, 'Just think of how much it takes for a person whose life is completely

together to just manage diabetes.'

Right.

Not diabetes and hypertension and potentially obesity.

Which, by the way, weight gain is exacerbated by the kinds of medications that manage some

forms of mental illness.

It changes your body's ability to gain weight, and then you're managing anxiety, and then

you're managing depression, and you're trying to hold down a job.

I would imagine there are reactions between the medication, too.

If you don't have the same person who is prescribing them or isn't aware of what's being prescribed,

and you have a patient who is having a tough time maintaining the different medications

that they are supposed to be taking, or that they are taking, that I imagine these interactions

become a problem.

Yes.

This is a patient centered medical home demonstration, and they were carried out all over the country,

and hopefully they will continue to be.

One of the key aspects of a medical home is to make sure the providers caring for a person

interact with each other and share medication information, and share information about hospitalization

so that a behavioral health provider, for example, knows when someone ends up in an

ER.

So that they don't have this situation where a person is being overmedicated or having

interaction.

Okay.

We're going to take a break, and when we come back, I want to talk about the specifics of

this pilot program and this grant that Missouri got, and I want to talk about why they're

getting a pilot program for Medicaid when they didn't accept the expansion of Medicaid

in that state.

We've got to take a quick break.

We'll be right back.

We're talking to Sheree Crute.

She has written a piece for the Washington Monthly, "How an Obscure Obamacare Provision

Is Quietly Saving Lives, and Money, in Missouri."

I'm Sam Seder.

This is Ring of Fire.

We'll be right back after this.

LaVar Ball Takes Shot at Kyrie Irving by Referencing Star's Late Mom - Duration: 5:03.

What's up, guys?

For Complex News, I'm Natasha Martinez.

LaVar Ball likes to speak his mind.

Sometimes, that can get him into trouble.

The latest LaVar Ball controversy comes from his Wednesday appearance on Undisputed.

When Skip Bayless, Shannon Sharpe, and Joy Taylor asked him about Kyrie Irving's comments

about him and his son Lonzo, he shot back by saying that Irving shouldn't be allowed

to criticize his parenting techniques since he doesn't have any kids.

This rubbed people the wrong way because Irving does have a kid.

His daughter, Azurie Elizabeth Irving, was born in November 2015.

She'll be turning two this year.

In the Instagram post of him sharing the news, he thanked his late mother who he named his

daughter after.

"Mommy, I gave her your name because I know you will bless her and our family the same

way you blessed me.

I love you."

https://www.instagram.com/p/-fa2jaMhO7/

But the worst was yet to come.

Elsewhere in the segment, Ball made some disrespectful comments about Kyrie's mom, trying to make

an argument that Lonzo's relationship with Lavar is different because he has two parents

and siblings.

Or something.

It only took a

few days before the internet showed their disgust:

TWITTER: I was really rooting for LaVar too after that Kristine thing.

But bringing upKyrie's deceased mother?

Lowest of the lows.

TWITTER: What Lavar Ball said about Kyrie wast asteless and deplorable.

I have been in his corner but speaking of his late mom is disgusting.

TWITTER: I was all for Lavar Ball.. but the comments about Kyrie's late mother, was way

out of line.

TWITTER: Kyrie should just put LaVar Ball on blast.

Totally inappropriate.

This isn't the first time Ball has been in hot water.

Ball made headlines during an appearance on Fox Sports 1 on the same day, where he got

into a tense on-air altercation with The Herd with Colin Cowherd co-host Kristine Leahy

over Big Baller Brand.

The

next day, Leahy said she has received death threats and offered her reaction

to what transpired:

It's probably safe to say that when Lavar speaks, he's going to send the basketball

world into a frenzy.

Has he hit a peak with his words on Kyrie Irving?

That's the news for now, but for more, subscribe to Complex News on YouTube today.

Player Interview : Stimpy! Best French-speaking player! (eng cc) - Duration: 14:13.

Hey everyone! Today we have a special guest, Mr Stimpy, who's one of the best French speaking player from Morocco!

Hello guys! Thx for presenting. It's a pleasure to work with you guys.

Pleasure is shared! So first question, can you present yourself!? What's your age and what do you do at the moment?

Im Stimpy, 15 years old, I'm in highschool and I am from Morocco. I play Brawlhalla for about 1 year and a 4 months.

That's pretty much it

Okay! Speaking of Brawlhalla, how was it when you started playing and how long did it take to make yourself a name in this game?

I discovered the game on a youtuber's teamspeak, he was oraganizing a game night, and the game chosen that time was Brawlhalla.

I really liked the game and started playing it on a regular base since that night.

First time i got good results was back in June 2016, where Blackie and I got top 8 in a BCS 2v2 tournament.

For 1v1, my first really good placement was back in October where i placed top 4 in a BCS tourny.

Good! That escalated pretty quickly!

I really like this game alot, and i found myself playing it for hours without even noticing it.

Once i placed third in a Friday Nigth Brawl and i realised that i enjoyed playing competitive and i almost play every tournaments since then.

Cool stuff! Was Brawlhalla the first game that you invested yourself so much in or there'are others?

Brawlhalla is the only game where i won money from esport tournies.

I played Fifa and league of legends a bit, but always in a cusual minded approach. Brawlhalla gave me the eSport feaver!

Perfect! Talking about that, what do you find interesting in paying at a competitive level.

Hum, it's a self satisfying feeling, when you place well in a tournament, you're proud of yourself, and of course making some money out of it will help me in my real life goals aswell.

But the adrenaline rush, seeing good results on the leaderboard and being recognize for it are the best things about all of that!

I understand, and in the game, what are your favorite legends, and what is your main if you have one?

I don't really have a main, but surely do have some favorite legends. Ada for example is still my highest character in the game, I like Lucian, Hattori, Scarlet, Diana and Val!

Okay! Are there legends that you really dont like?

Characters, that I won't use in tounaments or that I just don't like to play?

That you don't like to play

I find, hum

I Don't like the way I have to play when I use Orion

There's also, hum

Sentinel, I don't like to play him either

To be honest, I play almost everything, I'm ready to play any legend

If I wanna play a certain legend I'll do it

I see!

I might like playing a character on a certain day and may hate him the next day

I agree! I find it hard to stick to only one legend in this game

Totally! Especially in a meta that changes so frequently

That's true, In Super Smash Bros for exemple, I never had trouble finding a main. But in Brawlhalla, I wanna change all the time

I can't compare to other games cause Brawlhalla is my firts fighting game

It's hard to believe! Wow

Okay,

What is your best experience in Brawlhalla so far?

Winning the Legend Series Tournament was my best moment

That was the first S tier Tournament. I won against Maltimum after coming back from the loser bracket

So far, it's definitely my best experience playing Brawlhalla.

I think alot of us saw that final and that was crazy gotta say!

And what would be the worst moment for in this game, an unexpected loss or anything else?

I don't really mind losing, but let me a second to think

Ah yeah! During Winter Championships

I don't remove any credits from my opponents

I had huge internet connection issues during that tournament

And it got on my nerves a bit

So I lost ealier than expected cause of that

And it's definitely one of my worst experience in a tourny

Having lag issues in big events must be painfull yeah

You're from Morocco. You're a French-speaker but you play for Morocco. I believe that you're already at the second spot for eSports earnings in your country?

Yes i am! The first one is a WoW player so it'll be hard to take his spot!

Yeah, I'm proud of that cause there are not many Moroccan players in the competitive scene

Appart of this wow player, I don't really know anyone else in the Moroccan community

So yeah i feel honored to play for my country, a be able to represent it at a top level in Brawlhalla

Agreed, we don't know alot of Moroccan players in the competitive scene

Yup

So that's is really cool

And what are your goals and objectives for this year and 2018?

For this year, i wanna be able to qualify for the open BCX

I blieve that I'm already 2nd or 3rd in the circuit leaderboard

Ang if i go, i aim for top 4 in 1v1 and top 3 in 2v2

Great, and who's your partner in 2v2?

Lanz

Lanz was playing for Reason gaming with Ephi

But he's playing with me now and we have been successful

We placed top 3 and then top 4 in the Spring series

We had some troubles in the Spring Championship, but we'll keep getting better, i hope

Yeah, i think you won't have any problems. And any chance to participate in the Combo Breaker next year?

Maybe, we'll see

You don't have a sponsorship at the moment, do you?

No, not for now. But I'm searching and i'll keep looking to find a sponsor.

If anyone watching this video wanna sponsor a Brawl player, this is the best Moroccan player in this game! Make him an offer!

Do you have any tips to everyone that wanna get better at this game and maybe want to play at a competitive level?

To fix themselves goals when playing! Don't play just to play

When you start a gaming session, say to yourself. "What do i wanna accomplish today?"

And study the game. It's not just about using attacks

Every decisions that you make will have am impact in the match you're playing

So by studying and knowing stuff, you'll be able to have a bigger move selections to chose from

it helps you win alot more

And stay calm and don't get frustated cause it'll nullify your development and your growth

cause you won't see mistakes that you made

when we are tilting, what's the best tip you can give us? Taking a break or just keep playing and try to change our mind set?

Try to work on your mind set. If you really get mad, yeah, little breaks are the way to go. They can help alot

Recently, you created a coaching program, if you wanna talk about it?

I did! For anyone who wanna improve their gameplay, who wants to become a top player

I offer coaching services

You'll be able to find infos somewhere in the video

You can get in touch with me via twitter or discord, that's pretty much it!

Great! I think we talked about everything we wanted to

If you want to say anything else to our viewers and the French community Stimpy, you can

I played with alot of French players and you guys seem to be a good community

I feel like you help each other. Friendly people too, that i meet during tournaments and on discord

Really cool community!

And you guys too, KoM, clan Knights of Marianne, where i have a really good experience playing with your members

French top players like Daz are nice too, and they have a good future in this game

Yeah we believe alot in Daz, he's the best player from France

Great! Thanks alot for your time and for your answers!

For everyone who wants to follow Stimpy, If you wanna share your twitter with us and your twitch channel

You can follow me on twitter : twitter.com/realstimpy

I dont really stream at the moment but i plan on doing it in the future

Thanks everyone for watching, it was Jowser and Stimpy, see you soon :)
