This past Friday, Paul Ryan and the republicans and Donald Trump and everybody who has been

pushing for a repeal and replace of the Affordable Care Act were dealt a devastating blow when

the republicans couldn't even muster enough people to vote for their disgusting health

care replacement bill.

While this is a massive failure for Donald Trump, it's a massive failure for the republican

party, nobody is going to feel that failure more than Paul Ryan.

Here's why.

Paul Ryan has done nothing but talk about repealing Obama Care, coming up with a republican

plan for over seven years.

Every campaign that Paul Ryan has run since the enactment of the Affordable Care Act has

been against the Affordable Care Act.

That was his only campaign talking point ever.

That's how he became Speaker of the House, by telling everyone that John Boehner wasn't

tough enough on Obama Care.

He would be tough.

Put him in position.

He's going to make it right.

He failed.

He had one thing that he wanted to do as Speaker of the House and he couldn't even get enough

people to vote on it.

They could've voted, but it would've failed because they didn't have enough republican

votes.

He didn't even bother.

He gave up before he even really tried.

Donald Trump stepped in, tried to sell this bill to republicans and he failed.

This is Paul Ryan's failure first and foremost.

This is the only issue this man has been talking about, the only issue he's been Tweeting about,

for years, and he couldn't even do it.

What this means is that 2018 is going to be a very bad year for Paul Ryan, because in

addition to trashing Obama Care for years, he's also, in general, trashed President Obama.

In fact, in a Tweet this morning, he said, "We're going to work on repealing some of

these confusing labor laws of Barack Obama."

All of the responses to that were: What is your obsession with this man?

He's not the president anymore.

Why don't you work on leading?

That's not going to work as a campaign strategy for Paul Ryan in 2018.

He wants to continue to bash President Obama, to bash Obama Care, thinking that this is

going to get him reelected somehow, but it's not.

The American public knows that the failure of the American Healthcare Act, the republican

plan, was not because of democrats, it was because of republicans.

It was because of Paul Ryan.

It was because of Donald Trump.

You cannot run on a platform saying that this was the democrats' fault, even though Donald

Trump is out there expressly saying that today.

That's not the truth.

You cannot run trashing a president who left office two years ago and expect to continue

to win.

How do we know this?

Because the democrats tried that in 2010 and it didn't work.

You can't continue trashing the old and ignoring the fact that your party is not doing a very

good job leading, but Paul Ryan is going to keep at it.

He's going to keep talking about Obama.

He's going to keep talking about Obama Care.

He's going to keep talking repeal and replace.

In 2018, if he doesn't change his strategy, Paul Ryan will be the one repealed and replaced.

Better Chronic Management Through Value-Based Care | Humana - Duration: 3:41.

We have 10,000 citizens moving into Medicare each day right now.

If we're going to actually improve the health of these elderly patients, who will be us

in not too distant future, looking at outcomes and paying based on quality is something that

we should all be motivated to do.

I lost my father when I was young because he had a heart attack and previous to that,

he had had a stroke and he didn't know how to navigate the healthcare system.

We thought by going to an emergency room, him getting the care, he'd get the resources

and help he needed and truth be told, it was one piece of the puzzle.

He didn't follow up with anyone and in a short period of time, he died.

And, you know, I'm here to prevent that from happening to my patients.

It is unfortunate that Americans do not enjoy better health.

Part of it, I do believe, has to do with the legacy of fee-for-service medicine.

The environmental factors, whether it's where we live or our access to various services

in the community, things like water and food security, and those are actually determinants

of probably 70 or 80% of our overall health.

Only about half of people in this country take their medicine as prescribed by their

physicians.

And one thing we know about medication is if you don't take it, it doesn't work.

Here at Humana, we're making really great progress on our value-based work.

2015 results showed a 19% improvement in our quality and a 20% overall improvement in cost

compared to traditional Medicare.

We've done a number of research projects and it's very clear that one of the top

drivers of improving the way people take their medication is through the physician engagement,

that the patient knows that the physician cares, that the physician's engaged in that

conversation.

When you think about fee for service, I'm going to give people a prescription and it's

your responsibility, or you look at a value type of relationship where you say, "I'm

going to work with the patient to get their diabetes under control.

Or I'm going to follow up to make sure that patients understand why they need to take

their blood pressure medicine."

That's the opportunity that we have and that's what we have to do.

It is a holistic approach.

This is a culture change.

It's different than practicing transactional medicine, where I'm seeing a patient for

hypertension and treating the hypertension.

This is a 360-degree view of my patient.

It's important to recognize that this goes far beyond the direct delivery of healthcare

and in fact, must involve an assessment of those types of impacts from the community

and the resources that exist within the community to produce better outcomes and help people

live happier, healthier lives.

The person who really does well in this is the patient and if we are truly patient-centric,

then this movement to value makes sense.

Interview with CEO Rolf Habben Jansen | Hapag-Lloyd - Duration: 2:36.

Hapag-Lloyd finished its fiscal year 2016 better than many other competitors.

Joining me now is Rolf Habben Jansen, the CEO of Hapag-Lloyd.

Are you happy with the results?

Well, in the end, I think if you compare our results with many of our competitors our results is reasonable.

But on the other hand, in the end, it's still a loss so from that perspective it's not totally satisfactory.

What do you expect of supply and demand this year?

Well, supply and demand are starting to get closer together, because we see barely no new orders.

We see increased scrapping and there is also a fair amount of ships still being laid up.

So it is getting closer together but it will us still take a little of time before it's really in balance.

What else are the main topics in 2017?

That's difficult to say. I mean 2017 has been off to a good start, suddenly volume-wise.

And now we need to see whether the market recovers further.

There are suddenly some encouraging signs. But we'll have to see.

We have to work hard like every year, continue to work on cost, take care of the customers and then hopefully

we'll see a better 2017 than we saw in 2016.

The oil price has risen – what does it mean for your business?

Well, for our business it means that cost goes up, which especially in the beginning is always a bit of a challenge

because it takes some time before you can pass that on to the customer.

In general, I think to have an oil price that is not too low is actually good for our business

because it also prevents people from doing irrational things.

What are the advantages of the merger between Hapag-Lloyd and the Arabian shipping company UASC?

For us it brings a couple of things: It gives us access to the market in the Middle East.

It allows us to reach again a fair amount of synergies, definitely over 400 million dollars.

It gives us a better fleet because the fleets of both companies are very complementary.

And it also further strengthens our shareholder base

because we get two additional strong shareholders of Hapag-Lloyd.

There is a lot going on in your industry.

There are still smaller and midsized shipping companies

– do you expect more takeovers or bankruptcies in the near future?

We've seen a lot of activity over the last 12 months.

So it's difficult to see much more in the upcoming couple of quarters.

But I do think that there is going to be further consolidation.

But I doubt whether it's going to be in 2017. Probably a little bit later than that.
