(bell ringing)

- [Narrator] It's report card time at Bayside.

Jessie and Kelly's parents gave them gifts for their grades.

Zach plans to bribe his parents into forgetting

their disappointment with his incessant failure.

Lisa's dad gave her the credit card to buy something nice,

but she went bananas, and spent $386.

Zach kicks her while she's down.

Her real friends offer real advice: return the clothes.

Only Lisa can't, because they were on sale.

Zach offers his time tested strategy for shitty behavior.

- Enjoy it, then deny it.

- [Narrator] Lisa's actual friends offer actual advice.

Tell your dad the truth.

Zach says, pay your mom to tell your dad,

and put it all on the card.

The only plan in the room guaranteed

to make all her problems worse at the same time.

Kelly suggests paying her dad back.

Zach says, what about a $400 advance on her allowance,

continuing his hot streak of opposite solutions

rooted in no accountability.

Kelly says she can earn the money.

Zach says, this is a job for Zach Man and Jesus Christ,

does he just wear that all the time?

What a dork.

Zach asks Lisa if she's ready to make some real stacks

all day long.

- How?

- Hey, Lisa.

Sit down, look pretty, and trust me.

- [Narrator] Historically speaking,

nothing good has ever happened after a man says those words.

Zach begins his classroom prostitution empire.

He tells the teacher to write some bullshit on the board,

any bullshit, then greenlights Lisa's first horny client.

Lisa just made one whole dollar.

Only $385 more scarring sexual setups to go

before her debt is paid,

and she can start using those kissing bucks

to pay her extensive therapy bills.

Zach charged a whopping nine dollars

for two greasy nerds to double team her,

sending a clear message,

you can exponentially increase revenue the grosser you go.

Belding has an announcement about the annual charity drive,

but first, time for an earthquake drill.

Screech, emboldened by the many tickets Zach sold him,

harasses Lisa when they should be preparing

to survive a life or death scenario.

Lisa can't believe Zach would do this

without talking to her.

(audience laughing)

- I feel so, so cheap.

- Lisa, you're not cheap.

You made $36.

- [Narrator] Zach's already scumbagging

at a 12th grade level.

But Zach's not done selling Lisa out to perverts.

He says if she wants to make that proper bread,

she'll need to ditch the clothes,

and a great first step is selling the stuff she wears.

Zach has Screech rig every locker with her worn garments,

and kicks things off at maximum creepacity by selling

this teenager's lingerie to her most persistent,

unwanted admirer for two dollars.

Screech leaves to smellsturbate.

Zach slings shoes, wigs, anything you can use

to jack and sniff.

But when Belding almost discovers his stinky sale,

Zach has Slater distract him by saying

he wants to wear women's clothing.

Now Slater's cutting class to lie about his nonexistent

gender identity issues.

Zach's debased auction continues

by parading Kelly in swimwear.

When this unsurprisingly attracts a horde of degenerates,

Zach says he'll be buying it himself,

because he's a gentleman, and the only guy allowed

to strap that thing to his face like a snorkel.

Belding gets a call, interrupting Zach's orchestrated

sham combo, and catches Zach red handed.

Zach rats on Lisa under no pressure,

but Belding misunderstands

and thinks Lisa's donating these threads.

Zach takes no responsibility and gives away

all of Lisa's valuable stuff.

Zach says they only made $53, selling boots to sickos,

so Lisa had to do something more degrading

than getting sex trafficked and selling her underpants

for 75 cents: work a real job.

Lisa's tables are not tipping well,

and don't respect her, because they paid to violate her

hours ago, and now know what her feet smell like.

Zach tells her to stop being lazy,

and go get that loot.

Unable to gain control of her rude customers, Lisa quits.

Slater, a true friend, leads the gang

to work together to help her.

Looks like Lisa will be able to pay her dad back,

as long as Zach sells all those clothes.

Yeah, about that.

Zach says Belding thought they were donations,

and he couldn't tell him the truth.

He's allergic to it, but Zach's already scheming away

to un-donate the clothes to charity.

Lisa does not agree to Zach's plan

to rob the needy, and says she will finally confess.

Zach tries to talk her out of this madness.

Lisa thanks Zach for the fun day of sex work

and losing all her belongings,

but she is retiring from the game.

Lisa tells her dad everything.

He says, okay, sucks, but we're rich, so who cares?

Let's go hit Sizzler.

Only Lisa can't hit Sizzler,

because she is mentally replaying her heinous day,

as she will surely do for many years to come.

She says she earned $153.

Here it is, minus the cut she gave her other daddy.

And Jessie and Zach deliver some last minute dough.

Zach had Screech sell his body to science.

He'll put anyone on any corner.

It does not matter to Zach.

Then he does some awful prop comedy.

Does he just carry that around?

What a dork.

Let's review.

Zach Morris saw his friend in trouble,

and did nothing but put her down,

and offer her ways to make it worse.

Then, without any discussion,

sex trafficked her all over school one dollar at a time,

and sold her worn apparel to known deviants,

and when he got caught, gave everything away,

the antithesis of what he promised to do,

then put her down again, when she tried to do things

the honest way, in a work environment made untenable

by his wanton dehumanization,

then planned to steal back charitable donations

and did everything in his power to keep this innocent girl

from doing what she should've just done in the first place,

leaving her traumatized for life.

Nest Hello Video Doorbell | In-Depth Review - Duration: 8:55.

this is no ordinary video doorbell it

has some very sweet high-end features

like one of my favorites famously the

front door it's very cool and there's so

much more now I'm Ben from authentech

let's check it out

this is the nest hello a video doorbell

now to be honest it's been a pretty long

time since I've tested a product that

really blows me away in a few regards

and this nest hello actually does that

also connected to that I hate paying for

those monthly or yearly subscription

fees for a service that I think should

be free or included however with this

one I actually think it'll be worth

paying at that small yearly fee for the

high-end benefits that we get let me

show you what I'm talking about

setup is beautiful super easy and

straightforward I love it when the app

guides you through it step-by-step even

through all the basics so you can't miss

a thing and it's even easy for beginners

they also have an in-depth instructional

video which might be helpful I'll link

it to that down below the main steps are

download the app crear sign into your

cow and add device make sure your system

is compatible beforehand and one great

feature to note that mess hello even

works with electronic chimes which I

have just remember there's some other

video doorbells out there that do not

support this kind of lame just keep this

in mind wiring up the chime is fast and

easy they include this nest connector

which basically supplies a constant

power feed to the doorbell which is

needed

next remove your old doorbell determine

if you want to use the optional wedge

which I did I find it tilts the camera a

bit more towards the person's face for a

more clear view of who's at my door at

any time screw in the wall plate wire up

the doorbell it clicks right into place

add it to your home Wi-Fi and the rest

is setup through the app now enough with

setup let's move on to some of nest

hellos awesome features and what my

favorites are first that HDR video looks

fantastic I'm serious this is one key

area where a high dynamic range camera

is really needed under dark patios and

porches facing directly into crazy

bright skies it's the ultimate backlit

scene

and the last thing you want our dark

shadows where you can't see the person's

face at your door it's far from perfect

and there are still times it's a bit too

dark but a lot of the time day or night

all weather conditions it looks really

good on that note of camera remember

it's actually a 3 megapixel 2k color

sensor downsample 2 HD 1600 by 1200 30

frames per second if it was 4k that'd be

awesome but maybe we'll see that in the

next model at least for now it still

looks good the diagonal field of view is

a hundred and sixty degrees when you

have a camera next to your door you want

a super wide angle squeezing as much

possible in the frame this one is almost

perfect in my opinion the 180 degrees

width and height would be legit allowing

you to see packages left at the doorstep

or sneaky perps trying to peek through

the windows anyway this one is still

great and allows you to see one person

or a whole group of people that might be

up front standing close or far away now

let's go hit the doorbell and see how

fast it notifies and pulls up the feed

on our phone

now that is fast and I'm serious I've

owned a lot of these video doorbells and

security cameras and it pulls up at uber

fast which is so important I can't tell

you how many times I've been stuck on a

loading screen waiting for the video to

pull up by then it's too late the

delivery guy or whoever is long gone

massive well done done as for getting

this right the two-way HD talk and

listen sounds really good too there's

noise and echo cancellation Plus

pre-recorded messages hi there you can

just leave it Thanks just to say we'll

be right there

hi there no one can answer the door

right now we'll be notified you stopped

by once I was talking to my wife through

the doorbell as she walked across the

street and she said she heard me loud

and clear some other favorite features

of mine let's say someone's at the door

and in the settings you can determine

which activity alerts to be notified

people motion or sound whether they

press the button or not either way when

I receive one of those notifications on

my phone I slide down the drawer and it

acts

shows me a gif an animated snapshot of

the video clip this is small but crucial

and genius it allows me to determine

within seconds what course of action is

needed on my part I can ignore them or

tap on the notification to chat with

them over that two-way speaker or maybe

jump right into my smartlock app and

unlock the door another massive feature

it has facial recognition and voice

announcements so over time you tag faces

with names in the app and as it gets

smarter it won't just say someone's at

the door but I'll actually say their

name and this goes for app notifications

plus Google home announcements which is

very cool and helpful man is at the

front door I can't tell you how many

times I've missed a delivery guy at the

door because I'm Way in the back or I

can't hear the bell I'm away from my

phone this way it's announced on all my

Google homes spread throughout my house

so it'd be very hard to miss a massive

game-changing feature that some people

just don't realize nest offers 24/7 day

and night non-stop cloud recording not

just motion triggered events now yes

this takes a little bit of upload

bandwidth which you can determine the

video quality in the settings and yes it

also takes a nest aware subscription but

I'll touch on why I actually think it's

worth it in just a second so just think

about that how often does a motion

recordings start just a tad too late

missing the crucial beginnings of a

video clip or not record long enough

missing the ending or how about false

positive alerts on an aimless time line

scrubbing through too many motion Clips

nests has really nailed it here with

continuous cloud recording you never

miss the moment and I've never

experienced such smooth timeline

scrubbing before it's almost like

watching a full-on time-lapse on your

front door flicking through the timeline

as a breeze finding just the right

moment couldn't be easier the playback

speed is so fast it makes me want this

sort of technology on all my future

security cameras there's two different

timeline views and you can filter down

to just the type of events you want to

see like familiar or unfamiliar faces

doorbell rings and more you can

definitely still use this camera without

all this Nesta where fanciness but man

it sure on

some of its key best features another

helpful one is activity zones the camera

can let you know when it sees activity

in specific areas tap create zone drag

the outline markers color code and

rename and that's it easy-peasy that

familiar face feature is part of nest

aware as well okay so they offer three

different subscription tiers a 5 10 or

30 day for continuous cloud recording

plus to unlock those extra features now

yes this would start to add up very

quickly if we were to have a ton of

cameras spread throughout the home

obviously however for my rationalization

I only have one doorbell camera and a 5

day 24/7 video history plus all those

smart AI features unlocked at $50 a year

that's a little over $4 each month okay

so one buck per week I think I can swing

it if it's within your budget I

definitely recommend it

now one more very cool feature I didn't

want to forget they even offer an online

web app I'm always asking for this you

go to home nest comm log in and here you

can view the live camera feed fast and

easy playback we can even create a time

lapse and save or share our clips tweak

all your settings and here as well which

there's a lot of settings I appreciate I

think that's a consistent theme

throughout this whole process of playing

with and using the nest hello for over a

month now it's super well made and the

software engineering is some of the best

I've ever seen it's worth mentioning in

case you forget nest is owned by Google

for the pretty penny of 3.2 a billion

dollars back in 2014 and this is not

their first camera product also just

think about the software an AI and

learning that nests hello can tap into

Google for it really feels like a

premium product huge things to nest for

sending me the sample unit now it's not

super cheap it's currently 229 however

just compare that to some of its top

competitors out there like the ring pro

that's $250 so it's not that bad if you

want check it out all the links will be

down below let me know doubt in the

comments which video doorbell is your

favorite and why if you're new around

here consider subscribing for tech

videos like this every week until next

time let's live authentic

sBooster BOTW Power & Precision Eco MK II linear power supply - Duration: 12:42.

Already months ago the sBooster Best Of Two Worlds Power & Precision Eco MK II linear

power supply was announced.

But perfection takes time and finally delivery took place.

The sBooster products have been audiophile linear power supplies from the start when

they made their first model, the Best of Two Worlds, to power the Squeezebox.

In 2015 they introduced the much larger model named sBooster Best Of Two Worlds Power & Precision

Eco.

This is now being replaced by the MK II version.

The initial model was first available as 5 volts DC model but over time models became

available at any voltage between 5 and 24 volts DC.

There are two kinds of power supplies: linear and switch mode.

The linear power supply, like the sBoosters, have been around since the beginning of electronics.

Usually these power supplies were integrated in the devices they power.

But when the world became smaller and smaller, manufacturers wanted to conquer the other

continents.

That was not without complications though since voltages, mains frequencies and safety

legislations differ from continent to continent or even from country to country.

Especially for cheaper equipment it didn't pay to get local safety approval.

That's where the external power supply - the wall wart or power brick - comes into play.

The manufacturer of - let's say - a network player buys certified power supplies at the

right mains voltage and the right local approval from a local manufacturer, puts it in the

box with the streamer.

Now the streamer is a low voltage DC powered device that needs no certification while the

external power supply was already certified.

Then the switch mode power supplies came to market.

Due to their way of working they work at any mains voltage.

Asian manufacturers today offer these wall warts at any output voltage and amperage,

certified for world use and at extremely low prices.

It seems too good to be true.

And it is, at least for serious audio.

Switch mode power supplies switch the desired voltage on and off at a very high frequency,

often 200 kHz or higher.

Since there will be no current for the voltage is already switched off when a current even

considers to get going, a very tiny step down transformer can be used since there is not

risk of overheating.

A rectifier and filter then smears out the pulses to DC.

It's like laying drops of glue on a four by three and then using a spatula to even

it out.

The big advantage is that the small transformer is immensely cheaper than the transformer

needed for linear power supplies.

Another advantage is that such a switch mode power supply is capable of delivering power

instantly by design as where with linear power supplies it needs a clever design to achieve

that.

Switch mode power supplies have only one disadvantage: they are by nature extremely noisy and it

takes a lot of effort to reduce the noise to levels that are acceptable for critical

audio applications.

It can be done, as you might have seen in my reviews of the UpTone Audio UltraCaps LPS-1

power supply and I have compared the SOtM sPS-500 power supply to the sBooster in the

SOtM sMS-200ultra review.

About the same performance but both switch mode power supplies were more expensive.

And yes, both are specified to deliver a higher current.

But where the specified current for switch mode is the absolute ceiling, linear designs

still have headroom above their specified maximum current to cope with short peaks.

One last remark on switch mode power supplies: poor to average designs not only can have

heavily polluted DC output, they can also pollute your mains and so has negative influence

on other components in your stereo.

A notorious example was the first Super Audio CD player bij Philips.

That essentially was a very good Marantz design - Philips owned Marantz at that Time - but

the then infamous marketing department of Philips wanted a cheap switch mode power supply

to keep the price at a certain level.

If you only plugged in the mains connector of that player, somewhere close to your stereo,

the sound quality degraded drastically.

So even with the audio connections from the SACD player NOT connected, thus playing from

another source!

Therefore I always use mains filters for connecting switch mode power supplies.

Not to keep pollution from entering the switch mode power supply but to prevent the switch

mode power supply from polluting my mains.

Good switch mode designs don't need to cause this, but the number of good switch mode designs

is limited.

The basic architecture of the MK II has not changed from that of its predecessor.

Let's open both and show them next to each other.

There is little difference at first sight.

We see the power switch and the IEC mains socket directly mounted on the PCB holding

the twin stage mains filter and earth line choke.

These clean the input from high frequency pollution.

There are switches to set the mains voltage of choice.

These are set prior to shipping to the right voltage for the destination and a sticker

on the outside indicates the setting.

The transformer that follows is a custom made audio grade toroidal model.

The secondary side of the transformer feeds the rectifiers, the smoothing capacitors and

voltage regulator.

And there is a switch to select output voltages that are close together, like 9 and 10 volts

or 12, 13 and 13.5 volts, depending on the model of choice.

So what is different in the MK II?.

There is a different fuse holder, the colour of the PCB's differs and the cooling profile

of the voltage regulator appears to be slightly thicker.

That sounds trivial but things are not what they appear.

The transformer now has not only electrostatic shielding but now also HF magnetic shielding.

A transformer specialist told me that the magnetic shielding is not trivial.

The PCB's are redesigned to double sided while now 2oz copper is used, twice as thick

as is usual.

There also is a gold plated finish, as can be steen here.

The internal wiring is all high quality silver plated copper with Teflon insulation.

The output cable now has oxygen free copper conductors, braided shielding and a braided

sleeve.

The bespoke DC connectors are gold plated and all cables and DC plugs are soldered with

high quality silver solder.

And the production is brought back from China to Europe where more attention to detail is

given, according to sBooster.

Yes, the sound.

Since I used the 'MK I' versions for both my SOtM sMS-200ultra NEO and the Mytek Brooklyn,

I have replaced both with the MK II version and immediately had a problem.

How can I convey this experience to you in a credible manner.

You might have the idea that I know all, but I don't.

I'm just slightly ahead of most of you.

The changes that my setup 1 underwent over the last two years brought an immense increase

in sound quality.

From the sMS-200 to the Ultra, from the Chord Hugo to the Mytek Brooklyn, all sBooster powered.

Then the sMS-200ultra NEO and now the MK II sBoosters.

The sMS-200ultra was a giant leap, the Brooklyn a clear refinement and every other step another

refinement.

I already passed the point of stating that it all sounds analog.

And again the MK II's brings it another step further.

The voices are yet another step closer to perfection, transients are free and fast,

the background is yet further to the back, the spaciousness in some recordings is now

at a level that I won't play them when my brother visits since he has vaults anxiety.

And it's all credible, it sounds natural.

Low's got tighter too and I have never heard a glockenspiel this clean.

But we must see it in perspective, of course.

The sound of my setup 1 isn't that great due only to the sBooster MK II's.

It is an accumulation of quality.

Working on my setup 1 brought me a very well tweaked system where the slightest improvement

becomes clear immediately.

In my setup 3 the difference between the MK I and MK II will be a lot smaller.

I couldn't try this since I only have the 9 and 12 volt versions of the MK II and the

Raspberry Pi needs 5 volts DC.

In my setup 2 it must be already well audible.

Again, that's an educated guess.

It's all about matching qualities.

Putting racing tires on a Kia Picanto won't bring you a racing car.

And for those that own the same front-end: the biggest difference I got, was by replacing

the MK I sBooster on the Mytek, the one on the SOtM NEO also gave a clear improvement

but somewhat less.

I spoke to Wiebren Draaijer who designs the sBoosters and he told me that the build quality

of the Bulgarian factory is clearly higher and that alone does make a clear difference.

Add to that the better components and some other tweaking - I am sure he didn't tell

me all he did - did indeed improve the sound quality when used in my setup 1 - and I am

sure also in my setup 2.

Just one remark remains: although I love the company and their products very much, I am

not married to sBooster.

If another manufacturer thinks he has even better products or equal products at a lower

price, I am very interested in doing a review.

Must Follow This Precautions During PREGNANCY | Dr.Preethi Challa - Duration: 25:18.

hello viewers welcome to doctors Channel

today dr. preeti realized with us

consultant gynecologist hello Doctor

it's pleasure having a program thank you

doctor

if lady is pregnant what type of

precautions she should follow once a

patient finds out that she is pregnant

please do not wait go to the

gynecologist you nearby gynecologist

immediately and there are certain

precautions to be taken once she's

pregnant because the gynecologist will

also do a pregnancy test and then she

will confirm the pregnancy and there

will be certain things you have to

follow like diet you have to follow a

healthy diet and pregnancy this so as of

now there is nothing in particular that

a pregnant lady should avoid eating

certain kind of food you know back in

the day there was a myth where you can't

eat this kind of food you can't eat a

purple area or you can't eat a fruit but

there's nothing like that frankly now

medical textbooks there's nothing

written that you cannot eat this you

cannot eat that be healthy watch your

diet there are certain blood tests that

have to be followed and first three

months you'd be very particular about

your sexual activity try to avoid sexual

activity for the first six weeks of

pregnancy from the time you are planning

to conceive and once you have figured

out that you have conceived try to avoid

sexual activity till your first scan is

done because in the earlier stages of

pregnancy that is the first three months

of pregnancy it is a very delicate case

yeah so the mode of activity that

happens it made us a chances of abortion

or threatened abortion so we try to tell

our ladies stay away from sexual

activity for the first six weeks

following which a scan has to be done

and check if the heartbeat is there

mostly the heartbeat of the fetus comes

only after the six weeks of pregnancy so

following that you can be

relax once you see the heartbeat in the

scan and there are certain medicines

that have to be taken in pregnancy like

your folic acid medicine which is very

important please do not avoid your folic

acid medication if you have had a

previous pregnancy issues with any kind

of disorders in the baby especially any

neural disorders then always take a

folic acid preconception I know that is

before you planned pregnancy take a

folic acid tablet from the time you're

planning pregnancy yeah and once your

pregnancy is confirmed continue the

folic acid tablet and till your

gynecologist advises you that is mostly

till about the 14th to 16th week do not

miss the Foley cancer which is the most

important tablet for the baby's brain

development and neural problems yes to

take care of noodle problems then coming

to your verb lattice lot of patients

with all the blood test till the fifth

month which according to me is not

really right because starting of

pregnancy fee to the blood test then

what happens is you can pick up if there

is any problem for example say a thyroid

issue or anemia without discontinuing or

terminating the pregnancy

you can treat these causes and help the

mother have a healthy child by the end

of the pregnancy and by the end of nine

months and coming to after the third

month you have to take your iron and

calcium tablets also known as your

prenatal tablets depending on your

ethnicity like for example if you're an

Indian you'll have to take more amount

of calcium because our diet doesn't

provide that much calcium yes so please

make sure you have about half a liter of

milk in the morning half a liter of milk

in the evening what if possible if

you're lactose intolerance you don't

have to replace it with soya and please

understand that one litre milk gives you

one gram of calcium per day and you need

to have thousand mg of calcium that is

about one gram

- mhmm and then this don't expect you

you were drinking 1 liter milk and you

will get 1 gram half of it goes for a

waste into the bullet so what you have

to do is have calcium tablets everything

have ivory tablets everyday because you

have to maintain in for Indian ethnicity

you have to maintain your hemoglobin up

to 10 grams percentage okay normal

ladies when you are not pregnant 12 to

14 but when you're pregnant you have to

maintain it at 10 grams is a minimum

this helps in the healthy growth of the

fetus of the baby then when you go for

your antenatal checkups please make sure

your antenatal checkups are monthly once

up to the seventh month following which

seventh month you will have to go twice

a week month that is every 15 days and

once you enter the ninth month you will

visit your gynecologist

Wheatly and there is no set lot of

people a lot of patients ask me madam

how many scans see there are about five

scans which are mandatory scans recently

in how the baby is growing the brain

development to an extent and also it

will help me to tell you how the baby's

vital organs are okay okay

and so these five scans are mandatory

any extra scans that are going to be

done it will be only in an emergency or

if I am expecting something if I'm

expecting to see something like say

previous pregnancy she's had an issue

with the feet to some anomaly I have a

look for it in a certain time so do not

miss those five mandate three scans when

they're supposed to be done please get

them done body hygiene is very very

important in pregnancy

make sure your private areas are well

cleaned and kept dry young and travel

mostly we try to tell patients to avoid

traveling up to the fourth month because

following fourth months the pregnancy

moves a little higher into the pelvis so

what happens is it's

easier for patients to travel without

any side effects and first three months

to four months people experience a lot

of women experience a lot of nausea and

vomiting it is a part of pregnancy do

not avoid eating because a lot of ladies

I see they make anyways I'm throwing up

so might as well not eat which is very

dangerous

please take it in the stride and please

eat the more you eat the healthier your

baby is going to be and minimum weight

gain in the nine months of pregnancy is

10 to 12 kgs anything more is also not

good but no it's not it's it's okay but

don't gain too much weight because all

said and done your baby is going to not

be more than 4 to 4 point 5 kgs yeah and

the rest of the weight that you're going

to put on say you've put on about 20

kilos or 25 kilos you're going to be

stuck with it uh-huh which can lead to

diabetes which can lead to hypertension

so what you diet eat healthy eat well

your target should be to gain about 2

kgs per month these are the normal

precautions I give my patients there is

no restriction for work you can go to

work

you can work even about 10 oz a day make

sure you get about 8 hours of sleep in

the night and 1 oz rest in the afternoon

flat on a bed this is purely to avoid

back pains in post protein present

during the later months of pregnancy to

relax your muscles yeah these are the

normal precautions we give patients here

doctor in the first pregnancy if they

had any risk factors will it going to

continue for next pregnancy yes there

are certain risk factors which will

continue certain risk factors might not

continue may or may not for example say

if the patient had a abruption in the

first Pegman abruption is when somebody

gets a trauma or a very high blood

pressure the placenta separates and then

what happens is the blood supply to the

baby reduces which causes some kind of

problems to the fetus and delivery is

done

emergency there would be blood loss so

these kind of things is not necessary

that they continue into the second

pregnancy on to the next pregnancy and

certain conditions like infections or

vagina infections they might not

continue if taken now good if proper

precautions are taken in pregnancy

following a good hygiene body hygiene

and things most commonly what we see is

high blood pressure which we call as

preeclampsia yeah and then if they have

seizures it's gone as a clamp see and if

they have any diabetes these are the

normal things that we see it's called

gestational diabetes these are the

normal things that we see in pregnancy

when what people ask will it continue

and in certain cases yes it will

continue there is a good and a high

incidence of once you have it in one

pregnancy that it can happen in the next

pregnancy it increases by 10 folds from

a normal person who's not added to get

diabetes and a person who's had diabetes

in the first pregnancy it increases by

tenfold so the chances are there in that

case what they have to do is whatever

they've had previously in the previous

pregnancy there are certain blood tests

and certain precautions that they have

to take if they follow that in the next

pregnancy they can avoid it or it can be

picked up early without having any side

effects on the pregnancy doctor it can

be prevented it can be prevented certain

conditions like if they had thyroid or

BP or high BP or thyroid or if they've

had diabetes going earlier to the doctor

preconception and telling the doctor

your gynecologist that I'm planning to

conceive in that case what you can do is

go and tell your doctor that I've had

this condition in the previous pregnancy

what can I do there will be a pre

conceptional test which will be done

which will have your BP checkups which

will have your uric acid checkups and

which will have you urea checkup and

also will have your sugar check-ups if

this is all under control

then the chances of

having a smooth pregnancy with a close

watch on the patient it's good doctor

after the pregnancy is there any

contraceptive methods yes there are but

you can't use the usual contracept or

oral contraceptive pills that we

normally get over-the-counter cannot be

used for pregnancy when you're

breastfeeding the children see when

you're breastfeeding there's something

called as lactation amenorrhea which

itself acts as a contraception mm-hmm

okay but it is not 100% guaranteed okay

then again you can go on a pill called

as progesterone only pill which is very

safe when you're breastfeeding the child

mm-hmm but this should be started after

six weeks and this should be taken every

day one tablet irrespective of whether

you have bleeding or not daily one after

six weeks of delivery and you continue

it at the same time every day so the

most crucial thing is took take it and

the same time every day then we have

something called as copper T Mirena but

copper T is most commonly used in India

especially but the people can put it

even immediately post delivery but the

failure rates immediately after delivery

are very high yeah

so we wait for a few months and then we

give them a copper T option also these

are the contraceptive methods we have to

be taken immediately post delivery when

they're breastfeeding the baby young

doctor could you tell us why severe pain

occurs during the period C there are a

lot of reasons of patient can complain

about pain during periods yeah whenever

a patient complains a little bit of pain

causing uterine contractions is okay

manageable when a person comes and tells

me I'm having severe pain and the fact

that they have to go to a doctor that

itself means they have to get examined

that itself as a reason for them to get

examined they have to be investigated it

could be a fibroid it could be

adenomyosis or it could be even

endometriosis these kind of pains they

have certain

treatment each disorder has a certain

treatment fibroid and adenomyosis has a

certain type of treatment and

endometriosis has a certain type of

treatment for each one the previous

preliminary investigations have to be

done patient has to be examined as to

where exactly the pain is is it in the

back area is it in this lower abdominal

area and then treatment is done

painkillers are advisable there is no

necessity no study or anything that says

if you take painkillers during periods

it can affect your pregnancy or

fertility rate no that is a myth

actually most of the people are yes a

lot of people please I'm I request

everybody do not take the pain do not

bear the pain improve your quality of

life

there are painkillers non-steroidal

non-steroid and NSAIDs are there you can

take those which are absolutely safe and

the effect lasts only for the few are

for those few hours that you take take

the tablet and then it will be fine so

please take the painkiller under a

guidance of your doctor mm-hmm do not

take it on your own first get yourself

investigated examined and then go for

painkillers look so are there any

diagnostic methods are there to find

these cars yes yes absolutely

one by internal examination that a

doctor will do it is called a pearl

virginal and for rectal examination

secondly an ultrasound can be done to

pick it up and further you can go ahead

once you have four come with the

conclusion of their diagnosis you go

ahead with treatment depending on

whether the patient wants pregnancy or

the patient wants to just continue

without having any kind of paper

continue want to take treatment for the

pain or whether they want to plan

pregnancy if they want to plan pregnancy

and they have this kind of pain then

there is a different kind of treatment

mm-hmm and if they want to just reduce

the pain and then they're not planning

to conceive then they go with a

different type of treatment doctor

during the periods some ladies complain

with a chair breast discomfort white

occurs yes a lot of women complain of

breast discomfort before about 10 days

to 15 days before periods and it

continues and then it stops once they

start their flow then some patients

complain of only five days of pain

before periods and on the second day of

the period the pain disappears some

patients have ladies have pain

throughout 10 days before periods and

it's throughout the period and stops

only after the period has reduced in

that case you have to always rule out

any hormone imbalance or if you have

fibroadenoma of the breast also called

as the breast mouse when there is a

fibroadenoma of the press which is a non

cancerous breast lump these things you

can just take some there are some

medication for it you can take that

which will not reduce the size of the

breast lump but will definitely reduce

the pain that comes with the periods and

this can be picked up by a mammal scan

that is a breast scan yes or you can go

for a mammogram mammogram is a different

method of detecting breast lumps and a

mammal scan is a scan ultrasound machine

which picks up a breast lump doctor then

what is the treatment for lumps first

and foremost every woman should examine

her breasts once every month

self-examination there are a lot of

guides to examine your breasts if you

feel there is a lump please do not think

you are imagining it there if there is a

you feel there is a breast now go to

your doctor immediately and get a

Mammoth's can get an examination get a

mammogram whichever is feasible for you

and once you have picked up a lump any

kind of lump on your investigation go

for an FN

see FNAC is nothing but a fine needle

aspiration okay this will tell us

whether it is a malignant lump as an

ascetic arsenal of cancer lump or if it

is just a regular lung if it is a non

cancerous lump and it is causing a lot

of problem there are certain tablets

that can be taken for it if it is a

cancer Islam you have to go for

treatment immediately so whenever you

get these lumps there is no reason that

who gets a lump who doesn't get enough

if you have a family history if your

there is a family history of people

having breast lung it is more a reason

that you should get yourself checked

very on a regular basis doctor could you

tell us what is perhaps mere C now

because of the rising number of cancer

or cancer cervix cases that we are

noticing i pap smear is a screening

procedure it is a OB based procedure it

is a test it's not a procedure it is a

test that is done it hardly takes it

there's no pain it hardly takes any time

it is a smear of your cervix this is a

screening test to see if there is going

if you have any chances of cancer it

will pick up it is a screening test if

it says it is positive for malignancy

then you go for the next step if it is

negative for malignancy it is fine you

know you don't need any treatment but

also pap smear does because any

infections if a person has any

infections also pap smear picks it up so

regular pap smear is to be done before

sexual activity is started or a little

later also nearly once a pap smear is

mandatory for ladies please go for your

pap smear checkup every year once doctor

could you explain about the vaccines we

have something called as the cervical

cancer vaccine there are two types

Cervarix and the Gardasil that is a

polyvalent and quadrivalent trivalent

and a quadrivalent vaccine these are

given in three do

it over six months time over a period of

six months there are three doses for

this injections these can be taken

anytime from adolescent do your your any

age standing it and on ascent these

vaccines will definitely reduce the

occurrence of cancer and there are so it

only acts against the HPV virus human

papilloma virus this vaccine is only for

this kind of virus if the cancer is

caused by this it normally this vaccine

is for this HPV doctor is this cervical

cancers leads to modality yes

survival cancer obviously causes if it

is not taken care of

earlier the mortality rate will increase

mm-hmm doctor could you explain us about

the pregnancy died what type of food

they should take coming to your

pregnancy diet because when you are

pregnant you know the whole idea of eat

for two it doesn't really apply but eat

healthy and your normal diet your staple

diet whatever you eat every day that is

your breakfast lunch snack dinner that

should be taken along with which you

will need to take two glasses of milk

and normally people ask me patients ask

me what do i mix in listening

anything is okay from Horlicks to any

kind of dairy products for growth but

two glasses of milk is compulsory if

you're lactose intolerant we will have

to find a replacement for that for the

milk because you need above thousand mg

of calcium protein and post-delivery

when you're breastfeeding you need about

thousand 500 mg of calcium have a

healthy diet with lots of vegetables a

high-fiber diet if you eat vegetarian

food alone make sure you incorporate

some amount of green leafy vegetables

and high fibers if you eat

non-vegetarian please make sure you have

one egg a day it is good for your

protein

and also make sure you are eating good

amount of fruits this is where you get

your fiber from and try to avoid any

kind of aerated drinks a complete

avoidance of alcohol and cigarette

smoking no vine also in pregnancy doctor

pregnancy women can exercise yes I

normally advise my patients to start

their exercises from the fifth month

your regular exercise should not be more

than twenty minutes at a stretch when

you are pregnant this could be a leisure

walk to avoid any kind of excess

exercises should be avoided because it

can cause a lot of body cramping and you

can also know lifting weights I would

tell my patients to avoid lifting

weights prenatal physiotherapy exercises

are taught by physiotherapists these are

called prenatal exercises there is a set

amount of stretching of the muscles

which helps in easy normal delivery

these are something I normally tell my

patients to follow walking every day for

about at a stretch for 20 to 30 minutes

will make a lot of difference in muscles

to relax and other exercises like people

ask me can I do bridge

access a walking jogging no I do not

advise my patients to do a brisk walk

not do I love them to job I advise them

not to joke weights no I advise them to

go for prenatal exercises taught by a

professional a physiotherapist follow

only those and yoga I'm sorry I don't

have much knowledge about yoga

so I really don't tell my patients I

don't tell them to go for it or not to

go for it but physiotherapist advisor

and

exercises are definitely advisable

doctor finally what do you say about

congenital disorders congenital

disorders are something we really can't

help a patient when they come to us with

a scan saying there's a congenital

disorder we will have to see if this

congenital disorders treatable or not

treatable if it is treatable well and

good continue the pregnancy get it if we

can get the baby treated some of the

disorders can be treated when the baby

is in the uterus itself that can be

taken care of

a surgical pediatrician with an

ultrasound-guided doctors with a sow

knowledge astir these procedures when we

say congenital disorders the reason

mostly we get these congenital disorders

is one of the from the family tree

somebody has it it passes on to the next

generation having consigned - marriages

means when they are married in the same

families at the same bloodline examining

these disorders are highly so we are

very worried when somebody comes in to

us we are related before marriage we go

a little deeper into investigation okay

other than the mandatory

investigations we go with a little more

and we always ask the patients if there

is any ripken ssanghwa tea always go for

a karyotyping that is a blackness done

for the father and the mother the couple

goes for the blood test and then we can

see what are the congenital disorders

that can be passed on to the next

generation what is the percentage of it

being passed on so always if there is

any chance of consent - marriage make

sure the blood tests are done can we

prevent this doctor like I said

congenital disorders are not preventable

and we know no which pay which maybe

might have it which way we might not

have it so we can't determine before

Craig the pregnancy we cannot we cannot

tell which baby which pregnancy if the

same couple can have a one congenital

disorder baby and then they can have an

absolutely normal baby so there is no

particular couple where we can say that

a particular reason where only these

but this couple will have a congenital

disorder or this couple might not have

will not have a congenic a congenital

disorder can start in the first line

also thank you so much doctor that is

really useful information thank you I
